Bibliographic nitpicking: So Who Owns Death TV?

I posted this question as a comment on the main site (http://realitystudio.org/bibliography/b ... -death-tv/) but I thought the chance of getting an answer might be somewhat better here ...

So: Maynard&Miles, Shoaf, and Schottlaender all date the second ('expanded') edition of So Who Owns Death TV? 1967; however, the Lebel collage (one of the pages added for this second ed.) is signed: "J.J. '68", suggesting a later publication date. So: can anyone clarify? Or perhaps someone connected with Beach Books (publisher) or City Lights (distributor) remembers when this was published?

The Plymells do not remember specifics on the reprint including if they issued it. They sent out a few emails as well. I will tell you if anything solid turns up.
